It must be the area in front of the sump or the front of the sump itself, I have never seen a cross member that passed under the sump on a GM pickup.
I could not find a cross from the civy oil pickup and the military pickup but it does not extend much lower than the pump on the military engine...

Yes you have to run it NA, (or custom turbo) only the Van version (also center mount turbo) has the angled intake to head bolts, you cannot run a civy pickup motor with the center mount HMMWV turbo.

The output shaft will always be trying to spin in neutral and the engine running, there is a mass of components spinning on the input that that are riding on bushings/bearings and clutches that are wet with fluid on the output shaft, simple friction and fluid surface tension dictates the motion...

If it is a lifter it should be clear what side of the engine the noise is coming from you can verify buy listening to the valve cover on that side.

You may want to carefully inspect the front drive shaft, especially the carrier bearing.
Although possible it is unlikely your truck has enough miles on it to wear out the chain.

It sounds like a leaking lifter not able to stay pumped up, sometimes they will clear up on their own but if not you may need to replace it.
This is exactly why so many preach to do a FULL service on any surplus vehicle before riding around the neighborhood showing off their new toy, or thinking it is a good idea to hitch hike down to the yard and driving their new purchase home.
