I also have such a folder for one of my MEP802A generators.
It contains these forms:
DA5823 Equipment identification card
DA2404 Equipment Inspection and Maintenance Work sheet

This is DA Form 5823. This is the form that fits in the pocket on the Log Book cover. Back in the day, there was no DA Form 5823. It was simply a 3X5 card, filled out with the same info, shoved into the pocket.
Blank forms. If you need help filling them out, I might still remember how. 2404 was my great LOVE!!
The BIBLE for filling out these forms is DA PAM 738-750
This has been bugging me since I looked at this thread. Here is a list of what I seem to remember of being in the Log Book. I am sure I forgot something.
The official name for the Log book was Equipment Record Folder. It had in it:
