I would not worry about the DD-314. It is not a log book form. Its in the dispatch office. I hated to take care of the DD-314.

This is DA Form 5823. This is the form that fits in the pocket on the Log Book cover. Back in the day, there was no DA Form 5823. It was simply a 3X5 card, filled out with the same info, shoved into the pocket.
