I'm biased toward Cummins so I'd go with a 6bt. Plenty of room and good hp. You could use the stock trans but you may want to go with something a little heavier and better gear ratios. I haven't took any trans measurements so I'm unsure what would fit.

Water injection has been done by Jwaller (with good results according to him). There has also been intercoolers added (there's a thread around here somewhere with one on a 5-ton multifuel).
No need to worry about this in a multifuel. The IP is lubed and cooled by the crankcase oil, not the fuel as in some other IPs. As a matter of fact, these engines will run with the lift pump not working.
Interesting. I wonder if the 250hp number was a test for a short period or a real world operating test. I know you can get some crazy hp out of any engine for a short period (drag cars and pulling trucks/tractors) but they wouldn't make it to the store without blowing up in real world dirving...
IMO the problem with the multifuel holding up to high HP is the high compression. The first thing I would do to get more power out of it would be to lower the compression. In doing that you will take some pressure off of the rods, lower EGT, and would allow you to increase fuel/boost pressures...
Seems alot of people think if you turn up the fuel their truck will go faster. Your truck will not go any faster by turning up the fuel. Only way to get more speed is to change the gearing or mess with the governor (not recommended).
Turning up the fuel will help when pulling hills especially...
I have no idea what the max HP would be because to my knowledge nobody has ever put one on a dyno. Atleast they haven't posted it if they have.
You'll burn one up quicker than anything. Best thing to do is install a pyro and just don't go too much.
