Hood rub blocks on fender

I am trying to change a hood rub block on the fender next to the hood strap.

The bolts just spin. Are there riv nuts on the fender which are bad?

Is there a nut on the inside to hold with a wrench?
I looked and could not feel or see anything?

The nuts are inside the cab, not all that hard to get to, I change them all the time by myself and I am really old.

Just put one hand on the bolt and then try to touch that hand from the inside, should be able to do it with your eyes closed.
