Just put direct vacuum to the modulator, if it shifts you need a new vacuum regulator. When I worked for GM, I had a loop hose made I could just plug in place of the regulator to bypass it. If it still does not shift, replace the modulator.

Low voltage somewhere. My issue of the same was caused by loose battery cable at the reverse polarity relay. You don't say if it is an A0, A1 or A1R? Does make a difference for trouble shooting.
