My vote is for fuel filter as well. Sounds like it is getting just barely enough to do 45, but not enough to do any more.

My next to last M1008 had the same bolt plus another one loose when i bought it. I had to replace the pump because it leaked so bad where it had rubbed a hole in the back of it.
Have to disagree with you on this 'un. Never in all of tarnation (which is huge) have I heard of putting the thicker part of the wedge in back on the front leafs. If you look on some of them, they tell you on the wedge the thick part goes away from the truck.
