As far as the fuel issue I would suspect that the filter assembly is gone bad
When you pulled the filter I'm sure you saw that large round thing? That thing can leak air into the system and cause many issues. Go to Napa or whatever you like best...

So to start I have a couple questions for you.
1: does the truck have the factory square box style filter?
2. Is that tag that says 6.5 on the air cleaner?

I get some rattles and hums at idle from the interior of the truck. Usually I just slap the dash and all is good.
These trucks do vibrate and rattle a little.
They are not Cadillacs. Just saying
If it was my motor.
If It developed a cracked head and the bottom end was good to go I would take care the heads and go on.
How many miles are on the bottom end? How hard are you on the engine? These things come into play when deciding to do a full rebuild or not.

Rear diff oil change
I finally got around to changing the oil in the rear differential and checking out the chunk. I got the truck in my dad's shop on concrete and started working. I had my new gasket, my oil, permetex and tools. I cleaned up around the cover before I started pulling the bolts...
I think it is just air pressure. Try letting it cool off, then start is and let it run with no cap. That should help it work out the air without forcing coolent out first. Just make sure you have some coolent ready to go in it just in case a lot of air comes out and drops the level really fast...
