I guess just the basic process for registering an MV in WV. Anything I should know? Any different than registering a regular vehicle? What about a state inspection?

What kinda MV are you going to register? You need a title or SF-97, proof of insurance, notorized bill of sale and payment to DMV. State inspections are wherever in WV you decide to go for them, I prefer someone (Garage) I know.
If you are under 26000 lbs GVW then a standard WV inspection is fine (example cars, pickups). Over 26000 lbs and you need a commercial truck inspection.
