Help with data plate stamping

Do over...it didn't come out spaced right - ignore the periods.

I have a "Responsible Agency" data plate.
It has boxes to stamp in. My question is - what do you stamp in them? (The question marks mean blank space to stamp in)
Seems to me maybe a tech manual number goes in there...I just don't know!
