Are we referring to the rod that the mirror head attaches to? If so you can use all thread as a stop gap measure.

It is very common for that (threaded)rod inside the vertical pillar to rust away and the nut simply twist off at the first attempt for adjustment.
A piece of Ø1/4" "All-Thread", cut to 18-3/4 will help. Best if it was 1/4-28 UNF (fine thread)
