Does anybody have a picture of the proper winch control data plate that goes on the floor in the cab beside the winch PTO lever?

Finally ordered one from MVDATAPLATES. Sent check 4 Feb. Very nice data plate arrived in mail while I was out of town last week.

I guess my truck used to have a winch. It's got the door on the floor and the plate but it's painted over so I never knew what it said.
