Looks like your are new to the site so be ready for the search police. If you have a sprag t-case then you need that rod and arm, if you have an air shift case you can remove the arm but the rod stays. It tells the sprag t-case which way the truck is going in case it needs to kick in.
The XL's in my opinion look the coolest and they get the best traction, problem is they are a little tougher to find. The 177's are more readily available and probably the cheapest.
There is a pto that mounts on the back of the t-case and faces towards the rear. Found on fire trucks and fuel/water tanker trucks. There is a dual output one that mounts on the tranny but they are very tough to find.

I took my test with my 3/4 ton and 21K gooseneck, both DOT'd. I do not have the air brake endorsement but if you already have that I think you should be fine as long as the combo is over 26K GVWR and they are DOT'd.
Like I said it should have a switch under the dash to engage an air shift case. To make 100% sure you could crawl under the truck and look for an air line that runs along the passenger side frame rail that connects to a rubber line that connects to the front side of the t-case. If it has all...
If you have an air shift t-case there should be a lever under the dash gauges. If you have a sprag t-case it probably needs adjusting or just isn't working. If you have a sprag I recommend replacing it with an air shift t-case.
