Don't overlook an important point...this system requires a variable displacement piston (or some vanes) pump, pressure compensated, and is not a positive displacement (gear) type pump. Unless that OEM pump comes from an FMTV or HEMTT, it ain't gonna work. I'll grab the pad and go take a photo...

I'm working on this now myself. 3" makes it level to sagging. 4" has a bolt mount issue but would be level and 5" is rear high, but will (probably) level with a load.
If you are using the 105 springs and perches the top bolt mounting to the frame is an issue. Check measurements to see...

Yep. A3 wheels are considered "metric" and have a slightly different profile at the bead. I don't know if the bias tires you have have that same profile or not. It would be easy enough to make a cardboard profile and see if they are about the same.

Are you using beadlocks? I use the big bore Haltec on the A3 wheel. You can use the short valve stem for the back but I think it takes 5" to get out the hole when it's dish in (front). Why do you want CTIS covers?
