It's not starving for fuel cause it runs good at a higher rpm.
Possible air leak, but the rough idle would go away at higher rpm and then return SLOWLY at idle.
What is the idle RPM? Could it be as simple as turning the idle speed up a quarter or half turn?

Yep floors are different, If you look (IIRC) you'll see an outline on the floor where the tranny hump is, just cut the donor floor hump there (on the large size) and cut the crew cab alittle smaller makes swappin tranny's easy too!. Or just use a body lift.

The best way to do this is with a donor truck because beside the tranny itself your going to find that you need the clutch pedal assy.and clutch assy. shifter,flywheel crossmember,driveshafts ect. Also the starter motor is different from and auto vs standered.
