There are 2 seals in the hub area around the axle so the air has a passage to get thru to the valve. Hub has to be lubricated and wheel spins so you cant have a direct tube. Leaking seals allow oil in and/or fill air out and as the air passes in and out it picks up oil and distributes it places...
I'm not exactly sure. I have seen 2 different style lmtv ctis hoses from stem to valve. Not sure if there are different gen rims but over the course of 20 years a lot can happen. According to others, the covers do fit with minor modification though.
Fire it up use a infrared thermometer to see if the hyd pump assembly or anything else is getting really hot. I have moved big weight with mine without issue.
