What are you planning on operating with the wet kit? Any belt drive pumps I've seen aren't really suitable for things such as a dump trailer. The GPM of belt pumps will make a dump trailer painfully slow to dump.
State regs may have changed but not federal regs. There is no such yearly medical card requirement in TN. You are required in TN to maintain a medical card with a CDL and will be checked if stopped by LEO or when you go to renew your DL.

The M939 trucks with front winch have a trans PTO powering a Vickers pump. the stock pump is rated for 14 GPM @ 1200 RPM. In reality the pump is producing close to 25 GPM at wide open throttle. So the answer is yes, the trans PTO is more than capable for running the knuckleboom.
