PaFarmer, I will say that as long as you stay within 5 tons you'll be ok. The problem is when people put dumps on deuces they tend to exceed the 5 ton limit (myself included). It doesn't take much rock to get to 5 tons.

Setting the bed back will put more load on your rear tandems though. Ideally you want 60% of bed in front of the axle and 40% behind the axle. In stock form the deuce is about 55/45. I'd think if you went back much farther than stock you're front end could get light when heavily loaded.
I had a truck one time that would only start cold if you held the throttle to the floor until it hit. You would have to keep giving it a little throttle to keep it from dying until it warmed up. I turned the fuel up and then it started and ran fine cold.

Not if you don't have any air pressure. If you've got air pressure why would you cage the brakes? If the service brakes work there's no reason to cage them.

There are better ways to add PS. There is a kit out there that works just like the air-assist only it's hyd. I tried to finding the link but came up empty. When I checked it was kind of pricey but required the least amount of modification to make it work.
Frame width is the industry standard 34". There is an oddball every now and then but 99.9% of trucks (regardless of type and brand) will have a 34" frame width.
