all up a trans shop with the size/spline count of the output shaft and see if they have a stock one that will fit. else you may have to have a frankenstein shaft made.
Forgot to mention, one of the feeds has constant power, the others are switched from the ignition circuit, so if you want to be able to charge the phone with the truck off then be sure to get the circuit with power when the key is off.
Just get one of the "kits" to add one from the auto parts store. Then wedge yourself under the dash with a voltmeter and check the "add a circuits" on the far left side of the fuse panel. They fit a male spade connector. Just be sure to put an inline fuse on the feed line to the lighter. You can...

look into mounting the suburban 40gal tank in the original location, its been done on civilian blazers, although the brackets for the d-rings may be in the way, as well as the brackets for the pintle.
