i knew some end dump truckers in cal. that had 5th wheel rvs with standard kingpins instead ball receviers, on a long turm job, thay would bring there end dump trailer down, drop it , then go back and get there rv with the tractor, then pull the rv to a local rv park

also the tms show the military way, with all the spacific mil. tools, that does not mean thats the best way, easest, ect, let alone ALL the tricks that we have learned from each other
i have seen some custom goose necks that were a lot higher then standard, i have also seen goosenecks set up to be pulled by a standard 5th wheel, in that case a deuce tractor would work
as i said, i had to cut the nuts off my upper rods on the upper frame mounts, the lower upper rod mounts can be unbolted from the axle (in fact this is the only way to get it off the rod) and it will take a BIG hammer( i used a short handle 8# and a long handle 10# and repeted hits), when going...
