I have heard that the transmission is reliable if used within design parameters, but if grossly overloaded they will burn up fairly quickly.

I would say that they are worth less, but not by much. A significant percentage of those sold at GL have the box removed and a flat bed put on them. On the plus side, they don't seem to see as much use.

Litter is a synonym for streatcher. The military ususally uses litter in regard to ambulances as in the HMMWV 4-Litter Ambulance M997. I would love to see more pictures of how your litter racks work. I am most interested in the clamp as I may have some of them and didn't know it.
